Incident Reporting Guidelines

All Accidents must be reported to Axis Care. All work-related injuries or incidents, no matter how small or insignificant they may seem, need to be reported as soon as possible to an immediate supervisor or manager. All employees must fill out a report even if no medical attention is required. Failure to report an incident is a violation of company policy and can result in disciplinary action or termination. Please contact the following personnel if you need to report an incident/accident that occurred.
